In 1997, Pines Village Home Health Care, a state-licensed private duty agency, was established. Our home health care staff are able to provide daily assisted living care as directed by your physicians, such as personal grooming, escort services, medication set-up and reminders, and nurse case management. The security of knowing that your health and wellness needs are being met can make an immeasurable difference in life.

 
 

Home Health Care Services

 

As needs and preferences change, Pines Village residents may wish to add additional assistance. No matter the circumstance, a fall, sickness, or a short trip to rehab, Pines Village's state-licensed Home Health Care agency offers transparent assisted living packages and a la carte options. We endeavor to ensure security and maintain independence and optimum health while being conscious of resident resources. All Pines Village Home Health care services, including a la carte, receive case management by a registered nurse available 24/7.

Our professional team works with residents and families to learn what matters most. Each resident is unique, and a package of services will be customized for each individual.

 
 
  • ADMISSION VISIT
    $100.00 for the initial RN assessment and development of plan of services

    SCHEDULED HOME HEALTH AIDE VISITS
    $14.50 per medication reminder or assistance
    $14.50 per glucometer assistance
    $14.50 per meal assistance or reminder
    $14.50 per compression stocking assistance
    $34.00 per dressing/undressing assistance including personal care $34.00 per bath/shower assistance with dressing assistance

    LICENSED PRACTIAL NURSE VISITS
    $21.75 per medication administration (oral medications, controlled medications, eye drops, topical medications, insulin injections and nebulizer treatments)
    $54.00 per medication cassette set-up
    $21.75 per medication cassette adjustment (change to already filled cassette)
    $54.00 per intermuscular injection (not otherwise covered by insurance)
    $54.00 per nurse treatment (not otherwise covered by insurance)
    $54.00 per nurse visit for observation and report to RN or physician $57.00 per basic foot care/nail trim

    REGISTERED NURSE VISITS
    $76.00 per nurse assessment (required 60 day visit or for a change in condition)
    $76.00 per hour for case management family conferences, coordination with other providers and required medical plan reviews
    $38.00 per Home Health Aide supervisory visit (required every 30 days)

  • LEVEL I
    $2,242/Month (~ $74/day)
    For residents requesting scheduled daily assistance and occasional services as needed. If two residents sharing an apartment are both on Home Health Packages, they will receive a 25% savings on the Home Health Package of equal or lesser value.

    Individualized plan of services including any combination of the following:

    • Three meals a day included

    • Assistance with daily dressing/grooming tasks

    • Assistance with bath or shower up to threetimes per week

    • Assistance with compression stockings daily

    • Daily bed making with weekly bed linen change

    • Daily trash and tidy services

    • Weekly personal laundry

    • Weekly housekeeping

    • Medication cassette set-up

    • Medication reminders/assistance

    • Basic foot care (not otherwise covered by insurance)

    • Calls to physician or pharmacy for Rx refills

    • Calls to physician or lab for blood work or other testing needed

    • Access to a home health aide for emergency assistance 24 hours a day

    • Access to a registered nurse for consultation 24 hours a day


    LEVEL II
    $2,977/Month (~$98/day)
    Includes any combination of services from Level I and one or more of the following:

    • Assistance with incontinence products/skincare management

    • Assistance with bath/shower more than three times per week, if desired

    • Assistance with blood glucose/insulin management

    • Assistance with oxygen therapy, nebulizers, or CPAP Assistance with urinary catheter drainage bags

    • Administration of medications required by a licensed nurse

    • Assistance with simple wound services (not otherwise covered by insurance)

    • Complex case management of personal care needs by RN, including communication and coordination of care with all involved parties.
